Specialized Developmental Disabilities Program
Excellent medical care and a stimulating, enriched lifestyle are the
cornerstones of the program. Person centered planning reflects the pace of life, leisure and work preferences, opportunities for meaningful relationships dictated by each individual’s needs. Core values of the program

- Maintaining and improving physical and emotional well-being to support continued active participation in life
- Preserving functional independence to the degree possible using all therapies, adaptive equipment and assistive
technologies

The transition from ICF/MR to ICF/SNF level of care allows for
continued emphasis on the highest quality of medical and personal care assistance in conjunction with maintenance of functional skills and individual interests. A flexible daily schedule facilitates participation in a variety of
enjoyable activities such as cooking, horticulture, pet therapy, crafts, and intergenerational gatherings.
Who Will Benefit?
- Individuals who will be happiest with a self-paced, stimulating and enjoyable lifestyle. Compensated work
opportunities are available for those who choose to work.
- Individuals who need 24 hour support, daily nursing care, medication administration and assistance with activities of
daily living. Individuals may need restorative measures to maintain or restore maximum functioning.
- The Utilization Review Committee evaluates, using the FL2 and NC Level of Care criteria, the individual’s needs and
the care provided. The UR Committee recommends level of care changes. SNF care is available within the program when needed.
